It sounds to me you are laying to much into the charts. Charts are very relative things. They are jsut guidelines for the best hands to play in each postion. But I would have no trouble playing crap like 34o from MP with a raise if the players behind me were all rocks. You need to adjust your hands based on what the table is telling you, not some chart.
