First, I would reraise PF with a caller and completer. You probably have the best hand.

I would also srsly consider raising that flop, we want to build the pot if we hit our club. Turn is somewhat ugh but as played I definitely call.

Raise preflop. Continuation bet this flop after your raise on this flop 100% of the time.
As played on the flop, raise. As played on the turn, raise.
You are playing way way way too passive.
